Those offsets were for the 0616 patch. Use the patcher that Max Panda just linked above to patch your dll. Somebody also linked a pre-patched dll a few pages back that has wasapi and a few other things already patched for 0616 if you wanted to use that instead.

Enable shared mode wasapi through hex edit (mon took down the online patcher).
